For the purposes of this Privacy Policy, unless otherwise noted, all references to Upfront Healthcare microsites (or “microsites”) below refer personalized landing pages available to qualifying persons of Upfront clients in order to drive patient navigation and engagement. By using Upfront microsites, you consent to the data practices described in this statement below. We are committed to protecting your privacy and take great care in protecting your personal information.
Collection of your Personal Information
Upfront microsites receive personally identifiable information from covered entities (such as Healthcare Providers), such as your:
- Demographic data (name, address, etc)
- Contact data (phone number(s) and email address)
- Insurance data (insurance provider, insurance plan, member ID, etc)
- Medical data (doctors, care team members, prior and upcoming visit history, medical history, etc)
Upfront encourages you to review the privacy statements of your covered entity (i.e. Healthcare Providers or Health Insurance) so you understand how those websites collect, use and share your information. Upfront is not responsible for the privacy statements or other content on websites outside of the Upfront website.
No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
HIPAA and PHI
Under a federal law called the Health Insurance Portability and Accountability Act (“HIPAA”), some demographic, health and/or health-related information that Upfront collects as part of providing a microsite may be considered “protected health information” or “PHI.” When Upfront receives identifiable information about you from or on behalf of your covered entity (i.e. Healthcare Providers or Health Insurance), this information is considered PHI. HIPAA provides specific protections for the privacy and security of PHI and restricts how PHI is used and disclosed. Upfront may only use and disclose your PHI in the ways permitted by your covered entity.
Use of your Personal Information
Upfront collects and uses your personal information on behalf of your contracted cover entities to help you get the care you need when and where you need it. Upfront shares your response to microsites with your covered entity (i.e. Healthcare Providers or Health Insurance).
Upfront does not sell, rent or lease its patient information to third parties.
Protecting your Personal Information
The security of your information and data while using our microsites is very important to us. Our Upfront platform and microsites leverage technical safeguards to protect the confidentiality, integrity, and availability of your personal information including supporting Transport Layer Security (TLS)/Secure Sockets Layer (SSL) certificate technology for encryption.
Children Under Thirteen
Upfront microsites are not intended for use by children and children under the age of 13. The Upfront platform rules do not allow for microsites to be sent to persons under the age of 18. Caregivers or guardians, as designated by the covered entity (i.e. Healthcare Providers or Health Insurance), may receive microsites on behalf of persons under 18.
